DPOs REGISTRATION REQUIREMENTS
- A copy of registration certificate with authorized Registering authority
- a copy of groups constitution
- list of members or groups affiliated to your network for groups
- the most recent minutes of your meeting where office bearers were elected and resolution to join UDPK
- Brief profile of your activities where applicable

PAYMENT FOR SMALL AND COUNTY BASED DPOs
- for the first time registration payment will be KES 1,500/= Membership fee
- In addition to membership fee, you will be required to pay KES 1,500/= as annual subscription fee.

- The document can be delivered to our offices along waiyakai way opposite ABC place,Orthopadic complex.

- The document can be uploaded through the above application form
- Once the documents are received and reviwed and if successful,UDPK will communicate back to your organization advising on how to do the payments.
